How We Make It All Work

One of the most important parts of our person-centered approach is our Care Coordination team.

Each qualifying client is assigned a Care Coordinator who acts as a central point of connection, helping to coordinate services, communicate with your care team, and align your treatment plan with your needs and goals. Your designated Care Coordinator can assist with:

  • Scheduling and coordinating treatment plan meetings
  • Connecting you with additional resources
  • Supporting access to services such as medication management, pharmaceutical services, and dental care

Most importantly, they stay with you throughout your journey, making sure your care continues to reflect you.

ZVHC clients of all ages can be eligible for Care Coordination Services if they have a qualifying insurance plan and are receiving care through one of our Certified Community Behavioral Health Clinic services. To learn more about this service and if you qualify, give us a call at 507.289.2089.
