When helpful, friends, family, or other trusted individuals can join these conversations, so the people who support you understand your plan and can encourage you along the way.
• Connecting You with Resources:
From community programs to pharmacy and dental services, your Care Coordinator helps you access tools that enhance your wellness.
Who Can Access Care Coordination?
Our Care Coordination Services are available for people of all ages who have qualifying insurance and receive care through our:
• Recovery Programs
• Peer Support
• Psychiatry
• Therapy
• Adult Rehabilitative Mental Health Services
While our Care Coordination Services aren’t available for those currently receiving Targeted Case Management, Youth Behavioral Health, or Children’s Therapeutic Services and Supports, our team is always here to help.
